1996 Toyota Trecel: Jerks while driving and hard gear shifts?

This Problem is indicating for transmission inspection.The transmission needs to be checked.
The vehicle jerk while in drive mode and gears are hard to shift,the idle noticed will be high in this cases.

More than likely what you're going to find is that the cable that runs from the throttle body down to the transmission is out of adjustment. This will cause a harsh engagement any delayed shift point. The next thing to check is a throttle body itself make sure the idle air control valve is not sticking. Cleaning the throttle body would probably be a good idea also. As far as who can do this work any talented aftermarket repair shop should be able to check these things. If there is a dealer nearby they can take care of this also.
